Stereotactic Body Radiosurgery Using Volumetric Arc Therapy as an Alternative to High Dose Rate Brachytherapy Boost in Gynecological Cancer

چکیده
Background: The purpose of this study was to determine if stereotactic body radiosurgery (SBRT) using volumetric modulated arc therapy (VMAT) can be used to deliver a boost to the vaginal cuff in patients with gynecological cancer as an alternative to high dose rate (HDR) brachytherapy. Methods: Between January and July 2009, five patients with gynecological cancer were treated with postoperative radiation therapy to the whole pelvis with doses ranging from 41.4 Gy to 45 Gy. This was followed with a boost to the vaginal cuff with SBRT using VMAT. The total dose delivered in the boost ranged from 9 Gy to 21 Gy given in three fractions. Plan comparisons between SBRT with VMAT and HDR were performed for each patient. Results: After three years follow up (range 36 43 months), four of five patients (80%) are alive and well, except for one who expired of intercurrent disease after two years. Mild acute toxicity was seen in two out of five patients (40%). No chronic toxicity has been encountered. The dose coverage to the vaginal cuff using either HDR or VMAT was similar; however, the VMAT plan provided superior dose uniformity throughout the target, as well as better sparing of the bladder and the rectum at the mid to high dose levels. Conclusions: In this case review, target coverage, tolerance, and toxicity using SBRT with VMAT as a boost to the vaginal cuff were comparable to HDR brachytherapy. This technique could be considered as a potential alternative to HDR brachytherapy. Categories: Obstetrics/Gynecology, Radiation Oncology

Perineal recurrence of prostate cancer six years after trans-perineal brachytherapy
We report a case of perineal recurrence of prostate cancer 6 years after low-dose-rate (LDR) brachytherapy for localized prostate cancer. The most common approach to treat such perineal masses, including those occurring after prior biopsy or surgery, is local excision. We report the use of stereotactic radiotherapy with volumetric modulated arc therapy (VMAT) as a novel non-invasive, potentiall...

BACKGROUND MOSFET dosimetry is a method that has been used to measure in-vivo doses during brachytherapy treatments and during linac based radiotherapy treatment. Rectal displacement devices (RDDs) allow for safe dose escalation for prostate cancer treatment. This study used dual MOSkin detectors to assess real-time in vivo rectal wall dose in patients with an RDD in place during a high dose pr...

Local tumor persistence in the pelvis is the major cause of death in patients with recurrent gynecological cancer. Pelvic exenteration has been the only treatment option providing a chance for cure in selected patients. The aim of this study is to analyze the efficacy of stereotactic body radiosurgery (SBRS) in patients with recurrent gynecological cancer. Sixteen patients treated with SBRS wer...

Standard radiation therapy for cervical cancer consists of external beam radiation therapy followed by an intracavitary brachytherapy boost. When brachytherapy cannot be performed due to unfavorable anatomy or coexisting medical conditions, stereotactic body radiotherapy with the CyberKnife is another treatment option which is less invasive and can be performed in a shorter treatment time. We p...

Standard radiation therapy for patients undergoing primary chemosensitized radiation for carcinomas of the cervix usually consists of external beam radiation followed by an intracavitary brachytherapy boost. On occasion, the brachytherapy boost cannot be performed due to unfavorable anatomy or because of coexisting medical conditions. We examined the safety and efficacy of using CyberKnife ster...
